突變蟲穴是個到了遊戲後期才能進入的區域。在接到巫師的黑暗護身符的任務之後與科羅布斯對話後才能訪問。要到達突變蟲穴必須穿過下水道。
在完成所有收集包復興社區中心或是把社區中心改建成Joja倉庫併購買所有設施之後。玩家進入鐵路時會觸發一個事件，巫師會來跟玩家對話，並要求玩家去找暗黑護身符。
要找到暗黑護身符，玩家首先要和科羅布斯對話, 他會幫忙打開[[下水道]通往突變蟲穴的路。在裏面有一個放有暗黑護身符的箱子。將護身符放置在鐵路的洞穴口就能打開一條通路。
突變蟲穴不是特別大,玩家可以輕易的找到放着護身符的箱子。只需準備好對付怪物，那裏有大量的突變蟲和突變蛆。並且是唯一能釣到史萊姆魚的地點。這裏也能釣到鯉魚。在這裏釣魚是特別危險的，因為突變蟲會不斷產生和攻擊玩家。
